After you have successfully completed the StateFoodSafety.com™ Online Food Worker Course and Assessment, please present your certificate of completion and proof of lawful presence (per AZ House Bill 2102) at any of the five (5) Maricopa County Environmental Health Department Offices to have your picture taken and receive your official Maricopa County Food Worker Card.

You will be required to pay a $5.00 card fee at this time.
You will need a valid email address to register for this course. If you do not currently have an email address, you can get one for free online. We recommend Gmail, Yahoo, or Hotmail.
You must pass the Food Handler Course Assessment with a 75 % or higher to print your Maricopa County Environmental Health Department certificate.
