You must pass the Safe Food Handling Assesment with a 70% or higher to print your Certifcate of Completion.
   

You will need to present your Certificate of Completion at the Tulsa Health Department to take their brief waiver test to obtain your Tulsa Photo ID Card. At this time you will be required to pay the $10.00 health department fee.
The waiver test is offered every 4th Tuesday at 2:00pm. Please call (918) 595.4335 to preregister.

This course has been approved by the Tulsa Health Department.
